The independence of vulnerable people, and the treatment of others, is being compromised because of delayed transfers of care. This series of cross-cutting reports conclude that delayed transfers of care need to be tackled systematically across whole health communities. The report identifies specific scope for such systematic action, both short-term, and longer-term.
