Cardiff and Vale NHS Trust has developed a training programme for clinical staff to raise awareness and share best practice in effective discharges from hospital.
This course has been developed by an inter-professional group and has been led by the Intermediate Care Liaison Manager. There is a rolling programme of training, with one event each month, which is open to relevant staff. It is attended by about 30 staff every month. The training course covers a range of topics related to effective discharges and reducing the numbers of Delayed Transfers of Care, including:
As well as formal information-sharing, participants on the courses are given case studies to discuss and follow through to making decisions. This enables them to relate the formal information to individual examples.
Evaluation reports on the training have been very positive.
