Requests for information under the Freedom of Information Act should:

be in writing;
-
include the name of the person making the request;

include an address for correspondence;

describe the information requested.
Your description of information does not need to be perfect. However, being specific as possible about the subject matter and such things as dates is likely to help your request to be dealt with quickly.
If your request is for environmental information, it will be dealt with under the Environmental Information Regulations. These regulations define environmental information as going beyond information on the state of the environment so as to include information on such things as measures and activities designed to protect the environment. Such requests do not need to be in writing, but for clarity this may be advisable.
If your request is for information about you, it will be dealt with as a subject access request under the Data Protection Act. If you need further advice or other help in making a request, please call the Information Officer on 029 2032 0500.
In accordance with regulations set under the legislation, fees may be charged for meeting requests. If a fee applies, you will be informed, and the requested information will be supplied when the fee is paid.
If you have a complaint regarding the handling of a request for information, you should in the first instance write to the Information Officer at the address above.
If you are still not content with any subsequent response, you may write the Information Commissioner:
